Abstract

Rationale. In most surgical procedures, the need for blood transfusions is relatively rare. Despite this, tests for red blood cell phenotype and irregular anti-erythrocyte antibodies are widely used. The need for a more targeted approach to immunohematological testing in potential recipients of donor blood is discussed.

Objective. To identify patterns in the frequency of immunohematological testing and blood transfusions in a multidisciplinary hospital.

Methods. A retrospective study, using electronic medical records, examined immunohematological testing and blood transfusions performed in patients in 23 inpatient departments of the Pirogov Center in 2025.

Results. In 23 inpatient departments of the Pirogov Center, 11,841 patients were assigned immunohematological testing, 516 (4.4%) of whom received blood transfusions. The ratio of blood group determinations to red blood cell recipients varied between departments, ranging from 1.0 to 164.4, with an average of 45.9±50.5. Variability in this ratio is due to both the use of patient blood management and the active transfer of patients between departments for preoperative preparation and rehabilitation.

Conclusion. A “blood group determination to red blood cell recipient ratio” coefficient is proposed and discussed, which can be used as a benchmark for similar studies.
