Abstract

The article describes a new indicator — the volume/time ratio (VTR), which reflects the adequacy of cardioplegia during cardiac surgery. The method for its calculation is given. The possibility of calculating the required volume of a cardioplegic solution depending on the expected duration of aortic clamping with the help of a VTR has been demonstrated. The effectiveness and reliability of cardioplegia performed by infusion of reduced Custodiol volumes (1000–1500 ml) was shown. Prospects for optimizing myocardial protection based on a smoother Custodiol distribution with its fractional infusion during aortic clamping, have been outlined.

Keywords: Custodiol®, the effectiveness of myocardial protection, volume/time ratio, low-dose cardioplegia, single-shot cardioplegia, multi-dose cardioplegia.
